Online form had been working ok until about a week ago!
Now (vti-error1:SX\Saving Form results to Email will not
work unless either the "SMTPHost" or "SendMailCommand"
server configuration parameters are set. vti-error:IX\2)

I, too, am having a problem with emailing results in forms that just
recently has begun to appear. Here is what is happening to me (could the
issue be the same):
I recently moved my websites to a virtural private server. All of them have
multiple forms that send the results via email.
Now it seems that any of the forms which send the results via email do not
work for me from several browsers, isp's and locations, but do work for my
HSP when they test it.
When ever I complete the form and click on submit, I get an error that the
page is looking for:
http://www.mydomain.com/_vti_bin/shtml.exe/info_request.htm
info_request.htm is the page with the form on it. The form is supposed to
return with a confirmation page, success.htm which seems to be set up
correctly within the form properties.
All of my forms were working properly BEFORE I moved to a VPS. My HSP has
manually reinstalled the FrontPage extensions, that did not help. The form
does seem to work properly when the hsp tries it.
Anyone have any suggestions as what I can check or suggest to my HSP as to
what may be the problem? If you need to see the form, it is at:

Ask your host How they are testing it and to see the server logs for any info on errors

The host is testing it by completing the form and clicking submit. They
aren't having any errors and claim it works fine. Well except last night,
finally a tech said they had the same problem. Their latest "attempt" was
uninstall fpe and reinstall them. After this, the tech said it appeared to
be working correctly. My test is the same, it doesn't work.
My hsp's latest solution is to change to formmail to handle the forms!
I am getting very aggrqavated with this and my HSP as this isn't the only
issue I've had recently with them and it is taking days of emailing back and
forth to resolve issues.
I will suggest to them that they check the server log for errors.

Stefan: Today, I realized what is going on, don't understand it, but here it
is:
When my HSP is testing they are using the URL:
http://funddrive.com/info_request.htm and it works correctly.
When I test the page, I am using the URL:
http://www.funddrive.com/info_request.htm and it DOES NOT WORK.
What is the difference (other than the obvious www)? Where does a setting
need changed to it sees it the same?

My HSP finally kicked this up to level2 support. The problem was fixed by:
