Yes, you can issue users certificates even if you don't have Active
Directory -- but it is much more difficult. In this case you will probably
have standalone CA server.
One option (usually the easiest one) is to give users access to web
interface of your CA server. Here users will make a request for the
certificate. Once user submits the request CA administrator has to approve
(issue) the certificate on the server. Now that CA administrator has issued
the certificate user has to come back to CA web interface (from same
computer where he/she generated the request) and download an install the
certificate.
