I recently installed some security patches AND Win 2003 SP2, since then
Session_OnEnd is not firing. Does anyone know of any patches recently that
would be causing this?
There were 12 updates, plus the service pack and really don't want to
install one at a time to find the one with the problem. I also skimmed the
KB what the patches were fixing, but don't see that they were related to
IIS.

The Session_OnEnd regression was introduced in Windows 2003 SP2. There is a
fix available:
