Weird Problem When Opening Access

I'm having a problem that I've seen before on XP and am now seeing on
Vista. It started yesterday and I can't associate it with anything
that I've done to the PC, like installing software etc. I never got
to the bottom of the problem before and from memory it went away after
a hard disk reformat.
Sometimes when I double click on a database file to open it while
holding down the "Shift" key to bypass any startup code, nothing
appears to happen but an Access process is created in Task Manager and
an .ldb file appears in the file's folder. Access doesn't appear on
the screen or the Taskbar.
I then have to go into Task Manager and end the Access process and
then the database will start normally when I try again. The behaviour
isn't limited to one particular database file.
Has anyone else seen this? Does anyone have a clue as to what could
be causing it?

What maybe happening is that the last instance of Access didn't exit normally due to
the following error:
Access minimizes to Windows 95/NT toolbar instead of closing

And then when you do your double click and holding the shift key Access actually
reuses that instance of Access or does some other wierd stuff.
