TOC file does not merge correctly

I am trying to merge multiple .chm files and cannot get the child TOC to
merge correctly. I followed the instructions in the HHW Help on "Creating
links from one contents file to another", but when I compile and open the
parent .chm, I see the Heading I added as a book and a line under the heading
as another book, which looks like my include statement "icw.chm::/toc.hhc".
The child chm is being merged into the parent chm at runtime, which I've
verified by searching for an index entry and jumping to the topic in the
child chm. Any ideas on how to solve this issue are greatly appreciated.
