I just read the thread, so I hope I am not missing anything important.
You talk about a "php page", but the IMG code is HTML, so I suppose you
have the HTML tags in place.
I tried this

and it works fine.
Maybe you have some quotes missing, that are needed to have the right
syntax.

To get LDAP working in PHP for Windows, you first need to download the
ZIP package of PHP. Extract the ZIP package to some folder (do NOT
overwrite your PHP installation!). From the files you have extracted,
open the /ext folder and copy the php_ldap.dll file. Paste the file
into the /ext folder of your PHP installation. Make sure that the file
has appropriate security so that your web server can access it.
Then edit the php.ini file and remove the ; so that the line
;extension=php_ldap.dll

if you do not have the original line, simply add the new line before
the Module Settings section. Restart IIS or Apache (whichever one you
are using).

That will have an ext directory in it just like your current install of
php.net.=20

We have a web application which requires login into a database. At one
of the customer sites if once logged in and page is left alone the page
times out and requires a re login whereas when tested on my machinist
never logs me out even if I leave the page all day.
What could be causing the page to timeout? The whole website is created
using php and is running on apache. Database is MSDE.

You probably want to open Outlook on the machine that is not running PHP
(the browser machine) so you will need to use something else.
Try something like;
clickablevalue
This is html and is limited by the length of a valid URL (Get method), which
means the subject and body will need to be very short

If you really meant to open outlook on your server machine, check out COM
access in the manual, which will obviously need to be a windows server.

FWIW, you can basically do this with a standard HTML form where the action
is something along the lines of mailto:...
Can't remember if I've got this exactly right, but, basically your
tag would be something along the lines of:
enctype="text/plain">
Then, each of the fields in the form will be sort of included in the body of
that email.
